- Are you booting from an NVMe drive?

No, I have an intel controller where the OS drive is attached to.

- What type of test or tasks were you running before the issue appeared?

Issued a format command for one of the drive. Used this command for formatting "isdct start -intelssd 0 Function=nvmeformat LBAFormat=0 SecureEraseSetting=0 ProtectionInformation=0 MetadataSetting=0"

Did a reboot after completing the format command.

- Did this happen after a soft reboot, or was it a power removal during activity?

After a soft reboot

Thanks for your help.
